The quickest way to apply for Income Support is to complete a telephone application by contacting the appropriate
toll free # for your region. You also have the option of applying by mail by completing the following forms:

applicable) must complete and sign these. It is important to read the Release Form carefully as your signature allows the Department to contact other
agencies in order to verify information.
If you apply by telephone, a copy of the completed application will be sent to you for your signature. To ensure that it is processed in a timely manner, please sign and return it along with any requested documents to the following address:
Your application is valid for 30 days; if required documents are not provided within that time, you may have to complete a new application. You will be advised if further information is required. Once the application and documents are received, decisions on eligibility for benefits are normally made within five (5) working days.
